-Please bring your original passport for identification.
Applicants to Canada who do not have their passport can use:
Driver’s License (current with your photo), Provincial Identity Card, Refugee Document, or (under age 16) a birth certificate
-Please bring your glasses or wear your contacts for an eye exam
-Please bring your medications.
-Translator, if needed.
-If your name has changed recently: marriage certificate, adoption certificate, or name change form.
-We will take your picture on the day of your appointment.
-Cost of Immigration Medical Exam: (0-69yo) $200; (70yo+) $250; Furtherance $100.
-Cost of Chest X-ray and Labs are NOT included in this fee.
